In an attempt to use the required PHS398 pdf forms, I was not able to save any changes using ADOBE 9 (that we use for the full grant packages). I'm not sure if earlier versions of ADOBE would work, but just went ahead and used the provided word.doc forms for filling out page 1, 2, and checklist
It does appear that Adobe 9 does not allow you to save any changes, nor does Adobe version 8. The word.doc solution looks to be the preferred route if one wants to save changes to the document.
Based on the Notice #NOT-OD-09-056, one could request adminsitrative supplement funding up to two years. However, NHLBI requires the request to describe how the requested funds will be spent effectively by the end of the FY 2010 budget period. Does it mean one can request the administraive supplement ending by Sep. 30, 2010? i.e. the budget period will be 6/1/09 to 9/30/2010, 16 months instead of 24 months?
My project officer said that funds had to be spent by Sept 30, 2011, so my understanding is that you can request funding through that date, as long as it wouldn't go beyond either the end of your funded grant or the end of an already in place no-cost extension. That is, my impression is that you can't use supplement money extend the length of your already-funded/approved project.

NIH is advising that under the ARRA supplements applicants are encouraged to discuss budgets with their NIH program officer. Funding for administrative supplements to existing grants will be available from the Recovery Act funds in FY09 and FY10. Applicants requesting multiple year funding in FY09 may only be requesting funding for FY09 and FY10, and it is anticipated that supplement funding will be awarded within a short period after receipt of the supplement request. Requests selected for funding in FY10 will be limited to one year for the budget period.

Not knowing all the details of your situation and which IC the parent grant is under, I recommend the following:
1) Visit the parent grant's IC website and read their guidelines on Administrative Supplements (each IC will have their own guidelines). Make sure your request is eligible under the parent grant IC guidelines as well as the general NIH Supplement guideline to “increase the tempo of scientific research funded under the parent grant and/or achieving certain new research objectives as long as they are within the scope of the parent project."
2) If yes to the above, then briefly discuss the supplement request with the PI of the parent grant (requests are to come from the Principal Investigator of the parent grant).
3) Once you have a general idea of the supplement request (i.e. proposed activities, total budget request, etc.), contact the Program Officer of the IC which the parent grant is under and discuss the request. NIH encourages applicants to discuss budgets with their Program Officer.
The NIH program officer should ultimately tell you whether or not to go forward with your Supplement request.
After you’ve spoken with the program officer, please do post any additional information you can add to this thread.

NIH Supplement Guidelines do not specifically address whether or not references are included in the 5 page limit. I recommend contacting your NIH Program Officer for clarification.

Correction to the prior message about the 5 page limit. I posed this question to the Director of the Office of Extramural Activities at NIAAA (Dr. Abraham Bautista) and he said that references are excluded from the 5 page limit. Probably best to check with your specific institute.
I was told that the application should be sent to my IC by PI as an email attachment. My question is how the internal process will be done by UIC, how the cover letter be signed by the authorized organizational representative at UIC?
